This form should be completed where an individual has concerns with regards to the provision of ventilation in their area and any potential negative impact on the Universities Covid control strategy. Submissions will be sent to the Health and Safety Team and a response should be expected within 4 working days. This form is to assess areas that are occupied by 6 or more people for more than 15 minutes at a time as it is these areas that are likely to be of primary concern with regards to potential increase in Covid infections. The primary controls recommended are - To ensure you are up to date with any recommended vaccinations - That you your colleagues and students are undertaking regular covid testing - That all individuals are self isolating when required and reporting cases using the reporting portal Details of all of the above can be found on the University of Sussex Health and Safety Covid resources page. After completing this form the following actions may result - Information will provided on current ventilation rates and University covid control strategy - A check may be carried out on windows ability to open and information will be provided on suitable electronic radiators and how to procure them - A room occupancy limit may be set - The room may be closed and removed from use - The fitting of CO2 monitors in the room to monitor air flow. Occupiers will be provided with the information required to provide daily reports on CO2 levels that may necessitate changes in room use.
